精神科デイケアにおける双極性障害当事者によるピアミーティング

はじめに
　双極性障害は人間関係の悪化によって生活に困難を来しやすい疾患である^1〜4)。服薬においてアドヒアランス不良となりやすく再発契機と結びつきやすい⁵⁾。しかし，標準的な薬物療法を受け6か月間以上寛解状態にある双極性障害患者でグループ心理教育を受けた者は再発を減少させるとの報告⁶⁾や，双極性障害におけるグループ心理教育とセルフヘルプグループを組み合わせた精神療法は，再発予防効果や入院回数の減少効果がある^7〜9)との報告があり，ピアグループへの参加回数が多いほどQOLや幸福度向上に寄与する可能性が示唆されている¹⁰⁾。また，セルフヘルプグループは2018年のCanadian Network for Mood and Anxiety Treatments(CANMAT)¹¹⁾のガイドラインにおいて対人関係-社会リズム療法(interpersonal and social rhythm therapy：IPSRT)とともにセカンドラインの心理社会的治療¹²⁾の1つとされている。わが国の代表的なセルフヘルプグループ活動として特定非営利活動法人ノーチラス会¹³⁾があるが，医療機関による双極性障害に限った集団精神療法の報告は少ない現状がある。双極性障害当事者が精神科デイケア(デイケア)プログラムにおいてファシリテーターを務めるピアサポートグループの実践報告は我々の知る限りほとんどない。医療機関が双極性障害当事者とともに実践している，双極性障害当事者のためのミーティングについて報告することにより，今後の臨床実践において新たな取り組みの1つとなり得る可能性があるので紹介したい。
