物質使用障害治療とQOL

抄録　本稿では，依存症・嗜癖性障害の中で特に物質使用障害を取り上げ，その治療とQOLとの関係に関する先行研究を概説するとともに，物質使用障害治療に関する課題と展望を述べた。先行研究は，物質使用障害の治療がおおむね患者QOLを改善させる効果がある一方で，治療に対する反応性が乏しい者，ならびに，社会的孤立や併存精神障害を抱える者にとっては必ずしもQOLを改善させない可能性があることを示唆している。そのような者に対しては，ハームリダクション・アプローチや，そのより先鋭的な支援手法であるハウジング・ファーストがQOLの改善のみならず，物質使用の低減にも効果的であるかもしれない。最後に，わが国の薬物使用障害を抱える者に対する厳罰政策そのものが，当事者を社会的に孤立させ，QOLを低減させる可能性があることを指摘した。
