妊娠・出産をめぐるこころの問題

抄録　周産期の親子をとりまく生活環境は20世紀以降，急速な変化と大きな多様性を持つようになり，COVID-19パンデミックはその過程をさらに加速したと考えられている。今まさに育ちつつあり新たに世界に生まれ出てくる命にとって，どんな環境を整えればよいのかという現代社会の共通の問いに導かれ，世界各国で大規模なコホート調査が進行している。親世代の生活習慣と並んで，不安などのこころの問題もまた，周産期の段階から子どものこころと身体の育ち，健康に影響を与えている可能性が明らかになった。
　また出生後では子どもの情動制御や社会認知の発達を促す早期環境として親が一貫して養育的ケアを提供することは重要な側面である。精神医学におけるレジリエンス概念として親密な絆形成の過程が脳科学の領域からも注目され，affiliative neuroscience approachが提唱されている。ライフコースを通じたこころの問題の起点として情緒的絆は親子2世代の幅広いメンタルヘルスに繋がる疾患横断的な要因であり，予防的介入プログラムの重要な指標として多領域で臨床と研究のエビデンスが蓄積されている。
