思春期・若年成人世代がん患者の家族ケア

抄録　思春期・若年成人(adolescents and young adults：AYA)世代は日本ではおおむね15〜39歳とされている。当該世代の場合，患者の親も若年であること，社会経済的に安定していないことも多いこと，年少の子どもがいる場合があることなど，中高年層とは異なる特徴を持つ。たとえばAYA世代患者の親は，患者と同等の割合で精神症状を有することが報告されており，丁寧な説明を行うことで自責感を軽減したり，患者中心の意思決定を支援したりすることが役立つと考えられる。また，未成年の子どもには，抑うつ，身体愁訴，行動上の問題，認知機能や身体機能の制限などがみられることが報告されている。子どもは認知発達の途上にあり，病気に対する理解も成人とは異なる。子どもの発達段階や性格などの特性を考慮し，どのように患者の病気について伝えるかということを患者やその配偶者とともに考えることが，子どもに対する支援になる他，患者やその配偶者に対する支援ともなると考えられる。
