摂食障害（摂食症）の慢性化と回復の分岐点

　摂食障害（摂食症），特に神経性やせ症は本来，大多数が早期に寛解する精神疾患でありながら，10年以上の長期予後調査で慢性化率は10%を超え，さらに死亡率は10%に近い。そうならないために，まずは「18歳以下，病歴3年未満」という，家族をベースとする治療のエビデンスの範囲での寛解が目標である。次の分岐点として，おおよそ病歴7〜10年以上が慢性とされ，それを越えると重症遷延性神経性やせ症（SE-AN）となり治療は複雑化するので，そうならないように外在化などによって治療動機を高め，個別の慢性因子にも焦点を当てつつ治療を進める。一方，SE-ANに対しては，臨床的に意味のある完全主義（負けず嫌い）を理解し，摂食障害症状の改善を急がず，まずは生活の質の向上に重きを置き，絶望死を防ぐ介入が求められている。
