　児童思春期のうつ病の早期発見にはリスク因子の同定，ハイリスク群の抽出が有用である。リスク因子としては周産期エピソード（早産・極低出生体重児）や家族歴（気分障害や神経発達症），養育環境・心理社会的問題（逆境的小児期体験など），慢性身体疾患や神経発達症の併存などが挙げられる。これらのリスク因子に慢性的な感情行動調整の困難さ（irritability）を伴うと，うつ病を発症するリスクがより高くなることが知られている。児童思春期のうつ病の診断には発達段階による症状の違いに留意し，本人・家族からだけでなく，第三者からの詳細な情報収集に基づき家族機能や学校生活も含めた包括的なアセスメントを行うことが重要である。神経発達症とその周辺領域を主に診療する小児科医の視点に基づき，発達段階によるうつ病の症候学的な変化と発症のリスク因子について述べる。
