適応障害とうつ病性障害との鑑別

抄録
　適応障害に関して，かつては「ゴミ箱診断」などと揶揄され，特にうつ病性障害との鑑別については曖昧になっていた。特に，児童青年期のうつ病エピソードを過小評価する臨床医の診療姿勢や経験不足によって，うつ病性障害と診断される病状を適応障害としてしまう，あるいは正常範囲とみなしてしまうなど，誤診の危険性をはらんでいた。
　ICD-11の中で，適応障害は反芻思考と過度な不安によって，ストレス因もしくはその結果について先行する不安が生じると定義されたことで，より明確になった。治療についても現状は経過観察として対応していくが，若年者の一定数に自殺リスクを伴うことがあるため，注意深くみていく必要がある。そのためにも，適応障害とうつ病性障害についてはそれぞれの診断基準に従って鑑別を行い，適切な治療を進めていく必要がある。
