複雑性PTSD—疾患概念と診断基準

ICD-11における複雑性PTSD（CPTSD）はPTSDにDSO症状が付加されたものである。持続的・反復的出来事と診断との関係は確率的なものである。CPTSDの設置をきっかけとして，児童期虐待やヘイトの心理的影響についてCPTSDを参照しつつ，議論が活性化している。トラウマ的出来事を前提とした場合，DSO症状はPTSD症状との関係が強く，DSOの3症状は相互に関連している。DSO症状の意義については，今後の一層の議論が待たれる。累積した児童期逆境体験が心身の健康に与える影響は看過できない。累積逆境体験がある場合には，PTSDの出来事基準を満たさないいじめなどの体験の影響は軽視できない。診断基準の拡大解釈は推奨されないが，DSM-Ⅲ以降，心因，状況因についての議論が停滞していたことを考えると，CPTSDによって喚起された議論のさらなる活性化が望まれる。
