不登校と精神疾患

　不登校の支援において，その背景に潜む精神疾患の診断と対応は精神科医の重要な役割である。児童思春期発症の精神疾患は症状が非定型的で，診断確定に時間を要することも多いが，成人の精神科の知見が生かせる部分も少なくない。児童思春期の精神疾患の診断を試みるうえでの留意点を意識するとともに，その特徴を把握することで，一般の精神医療の場でも可能な支援がある。治療に関しては，統合失調症，双極症Ⅰ型では薬物療法，うつ病，不安症，強迫症，摂食障害では心理社会的治療が中心となる。軽症例では疾病教育主体のアプローチで改善が得られるケースも多く，本人，家族との共同意思決定を心がけながら試みるとよい。不登校の支援の目標は，再登校することではなく，本人なりにwell-beingを追求するための基盤作りであり，医療や教育はともにこれを支える柱の1つである。限られたリソースの中，最善を追求し続けることが，よい治療関係ひいては回復の礎となる。
