双極症における難治例

　双極症とは，エピソード性の気分症で，経過中に躁・軽躁・抑うつなどのエピソードが発生することで定義されるが，標準的治療が奏効しない難治例もしばしば経験される。難治性（治療抵抗性）の診断のために，まずは診断の誤り，併存する身体疾患や他の精神疾患の有無の影響，服薬アドヒアランス不良，忍容性不良による治療継続困難，薬物代謝などの見かけ上の難治性を評価する必要がある。治療に関しては，薬物療法・非薬物療法ともにエビデンスは不十分である。一部のガイドラインでは治療抵抗性双極症，急速交代型双極症の治療に関する推奨はあるものの，本邦の保険適用の薬物療法ならびに非薬物療法の候補は非常に少なく，本邦適応外使用薬あるいはサプリメント，本邦未承認の薬剤が多いため，実臨床への対応には注意を要する。
