難治性うつをどう考えるか

　なかなかうつが治らない病態について，これまでは「治療困難性うつ病（treatment resistant depression：TRD）」と呼ばれてきた。薬物療法をいくつか施しても効果がみられないから別の治療をという発想であったが，最近では，評価尺度を用いて症状を正しく評価したうえで診断の再検討を行い，さらに当事者のパーソナリティや発達傾向などの素因も加味し，当事者と共に，対処行動を含めた治療アプローチを考えていくという「難治性うつ病（difficult to treat depression：DTD）」という考え方に変わってきている。この考えは急性期での反応の乏しさだけでなく，急性期に見られた「反応」が持続しない例についても含め，慢性疾患モデルで考えるというものである。
　DTDは，対応についても「治療」という考えにとどまらず，転帰不良因子として併存症などをコントロールし，心理社会的機能を改善させ，ゴールを当事者と話し合い，さまざまな対応に加え，当事者自らがセルフケアを駆使するというものである。
　なかなかうつがよくならないと，当事者や家族，そして治療者自身も諦めてしまっていることが多い。さまざまな観点に立って諦めずに多様な取り組みを行うことを強調したい。
