心的外傷後ストレス症（PTSD）難治例の臨床

　心的外傷後ストレス症（posttraumatic stress disorder：PTSD）は難治化しやすく，その要因として以下のようなものが想定される。第1に，PTSDの治療薬は選択肢が乏しく，そのうえ特別な技法による心理療法が第1推奨治療とされているため，一般的な精神科臨床においてやや扱いにくいと考えられる。第2に，種々の精神疾患を併存している例が多く，そのようなケースではPTSDに加えて併存疾患の治療も必要となるため治療が難しくなりやすい。第3に，トラウマの性質も予後に影響し，特に持続性・反復性トラウマのケースでは難治化する傾向がある。本稿では，PTSDの難治例にみられることの多い上述の各要因を検討し，それに基づく治療法や対処法を概説する。PTSDの難治化を防ぐには，第1に基本的な対応と治療が肝要であり，そのなかには日常臨床の範囲で実施できることも多い。PTSD難治例を大幅に減少させるには，これらの臨床家の努力に加え，新規治療薬開発や効果的な心理療法の普及も重要となる。
