摂食障害難治例に対する治療的介入とは

　摂食障害の難治例は重症遷延性摂食障害（SEED）や重症遷延性神経性やせ症（SE-AN）と呼ばれており，主に罹病期間と失敗した治療回数が難治の目安である。難治例の場合は特に，症状の回復を目指す医学的リカバリーだけでなくパーソナル・リカバリーの視点も重要であり，治療方針については患者とよく話し合って決定するべきであり，状況に応じて治療契約を見直す必要がある。一方で，回復する例のあることも忘れてはならない。また，現代における摂食障害の精神病理や症状の背後にある心理的な課題についても検討する必要がある。治療継続のためには海外の方法を取り入れるだけでなく，デイケアなど，既存の方法を活かすことも考えたい。支援体制の充実に関しては近年設置された摂食障害全国支援センターと摂食障害支援拠点病院に期待したい。さらには，難治化する前に予防・早期介入することが喫緊の課題であり，そのためには教育現場との協働が不可欠である。
