「ボーダーラインパターン」というシナリオを想定してみる

　本稿では，治療者がいわば「医原的に」ボーダーラインパターンを難治性にしている側面を，シナリオという言葉を使って考察した。シナリオは色眼鏡の働きをし，治療者はその色眼鏡越しに患者を見るようになる。シナリオに影響されるのは治療者だけでなく，その色眼鏡で見られる患者のほうも知らず知らずのうちにシナリオに沿った言動をするようになる。ボーダーラインパターンの難治性の中には，こうしてできあがっている部分がある。ここで重要なのは，こうした部分はボーダーラインパターンの治療では不可避的に生じるということである。したがって，治療者に求められるのは，シナリオの存在に気づき，それを外して患者との直の交流を回復させることである。そのための方策として，心理療法を学ぶことで虚心坦懐に患者の話を聞き患者の理解に努める基本姿勢を身につけること，スーパービジョンを受けること，症例検討で率直なコメントを受けることなどがある。
