薬物治療においての「治療を終える」

抄録
　統合失調症は，「寛解」しても「治癒」は難しい病気であり，抗精神病薬の継続が必要であると考えられている。抗精神病薬の再発予防効果には高いレベルのエビデンスが存在する。代謝性副作用についても，長期服薬による生活習慣や受療行動の改善によるメリットがデメリットを上回ると考えられている。抗精神病薬中止の前段階にもなる間欠投与については有用性が否定されているが，減量については意見が分かれている。治療終結を希望する患者の気持ちは強いが，精神科医の側にその希望に向き合う準備ができているとは言えない。再発リスクの低い抗精神病薬の中止方法，再発リスクの低い患者群の同定に関するエビデンスの蓄積が必要である。
