身体症状症の治療終結—その可能性を高めるには

　身体症状症（somatic symptom disorder：SSD）の治療終結に関する研究や論説はほとんどない。このため，他の疾患からの類推や筆者の臨床経験から論じた。まずは，SSDにおいても治療終結を目指すべきであり，その前提となる寛解は，治療を徹底的に行えば不可能ではないと考えられる。次に，真の治療終結のためには，再燃・再発の防止が必要である。強迫によるSSDでは再発が中心で，それも強迫症よりは少ない印象がある。十分な期間の薬物療法，強迫的な認知や行動の修正が望ましい。不安によるSSDでは再燃が多く，特に減薬中止の直後に生じやすい印象があり，たとえ薬物療法の継続期間を延長しても生じうる。慎重な減薬方法，ベンゾジアゼピン系抗不安薬の使用を控えることを含めプラセボ効果が生じないような配慮，「ストレス恐怖」を含む病態についての適切な心理教育，症状の出現を回避するのではなく症状への曝露を目指すようなアプローチが必要である。
