摂食障害患者の栄養食事療法

　摂食障害は，思春期の心理社会面や身体面の健康に大きな影響を与え，若すぎる死のリスクの増加と関連している。ほとんどの思春期患者は問題を認知できず，治療の実施がとても難しいケースが多い。したがって，長期間の悪影響を避け早期に効果的な治療を行うことが重要である。摂食障害患者の栄養食事療法において治療のゴールは体重回復となるが，摂食障害の精神病理を維持するメカニズムを修正していかなければ，治療を継続することは難しい。認知行動療法改良版（enhanced cognitive behavior therapy：CBT-E）は，思春期患者の治療モチベーションに注意を払い続け，患者自身が行動を再びコントロールできるよう支援し続けるという考えに基づいている。摂食障害患者の栄養食事療法においては，CBT-Eを十分理解した管理栄養士が治療に参画すべきであり，医師や心理士と綿密な連携をとりながら患者や家族との良い治療関係を構築し，低栄養の改善・体重回復に努めることが重要である。
