老年期精神障害の脳器質的背景—レビー小体病とVLOSLPに着目して

　臨床的要請に基づき，かつて精神病理学的観点から老年期精神障害の諸類型が提唱された。DSM診断基準が本格的に普及した1980年代以降，広く知られるような新たな類型は提唱されなくなった。「老年期サイコーシスないし統合失調症性の病像を示す臨床的一群」について国際的合意形成の観点から2000年に国際会議が持たれ，「40歳以降発症」の統合失調症（late-onset schizophrenia）と並置して「60歳以降発症群」はvery late-onset schizophrenia-like psychosis（VLOSLP）と命名することが決定した。VLOSLPという臨床単位の背景病態は単一でなく，近年は「緩徐進行性の神経変性疾患の表現型の一部」として捉えるアプローチが主流となり，本邦からの報告を含め知見が蓄積されている。本稿はこれらの概略を述べた。
