強迫症（OCD）における気づきと病識（洞察）—閾値下強迫症状との関連性をふまえて

　元来，強迫症状には，不確実性や過剰な責任感などに基づく不安への防衛反応的側面があり，一般人口の約30%に生涯有症を認めるなど，かなり普遍的現象と言える。しかし中には，何らかのきっかけで制御を失い強迫症（OCD）に移行することがあるが，その場合，より早期に本人や家族が気づき，精神科に相談や受診し，適切な治療を受けるといった未治療期間（DUI）の短縮が重要となる。しかし，OCD患者の現状は，発症初期でも必ずしも十分な洞察を有さず，また有しても安定的には維持されずDUIも概ね7〜8年と長期であり，適切なタイミングで適切な治療がなされているとは言い難い。このため今後は，一般への啓発活動，他科との緊密な連携などがますます重要になると考えられる。
