身体症状症における病識

抄録
　身体症状症の患者は，病識が乏しい。これはいわゆる神経症に含まれる他の疾患と比べ，際立った特徴である。その原因は，主症状が身体症状であること，強迫的とらわれ，認知機能障害，身体化における疾病利得などが考えられる。病識が一見ありそうな患者も，真の病識はないことが多く，この場合は治療がかえって難航することがある。病識が乏しい場合，治療導入の困難さ，ノセボ反応による難治化や副作用の訴え，治療中断などが生じやすい。病識が乏しい患者に対しては，治療者は能動的なスタンスをとるべきであり，家族のサポートも利用すべきである。さらにはピアサポートの利用も考えられる。
