物質使用症の気づきと病識

　「依存症」とは，物質使用のコントロール障害を指す疾患概念であり，アルコールをはじめとした精神作用物質により発症する。物質使用症はWHOのICD-11に収載された疾患で，物質依存だけでなく有害な使用パターンなどを含む包括的な症群である。物質使用症における病識の欠如は，治療への抵抗につながり，トリートメントギャップを引き起こす要因となる。物質使用症の1つであるアルコール使用症は，再発率が高く，健康への影響が深刻である。アルコール使用症の治療においては，当事者の病識を高める工夫が重要であり，そのための取り組みとして，自助グループや認知機能改善プログラムなどが試みられている。物質使用症の治療において，治療者は，当事者の気づきや病識を促す一方で，病識の獲得に固執しすぎず，共感的・支持的な姿勢を示しながら，当事者—治療者関係を構築し，治療継続を促すことが重要である。今後の課題として，物質使用症に対する社会的な偏見を取り除く普及・啓発活動が挙げられる。
