　アディクションは，単なる快楽追求や意志の弱さによる行動ではなく，生物学的，心理的，社会的要因が複雑に絡み合う疾患である。本稿では，アディクションの神経基盤に焦点を絞り，反射系と衝動系の障害を中心に概説する。アディクションは，報酬系の過剰活性化と実行的認知システムの弱体化により特徴付けられ，これにより嗜癖行動が維持される。また，アディクションには脳の特定領域の機能障害が関与しており，これには意思決定，記憶，注意制御などが含まれる。さらに，物質依存と行動嗜癖の相違点，アディクションのサブタイプに関するパスウェイモデルの紹介，アディクション治療における認知行動療法や動機付け面接の選択の際に参考となる臨床像と神経基盤について説明する。アディクションの脳科学への理解を深めることにより，患者の言動を理解し，偏見を減らし，適切な治療選択へ役立てられる。
