がん医療における精神医学—サイコオンコロジー

　がんは1981年以降わが国の死亡原因の第1位であり，年間100万人以上ががんと診断されている。がんは最も頻度の高い致死的な疾患であることから，国民病という位置付けがなされており，わが国のがん対策基本計画においても，がん患者の精神症状対策が重要事項として盛り込まれている。がん患者のおおむね半数には何らかの精神医学的診断が認められ，患者の生活上の悩みとしても精神症状が最も多い。がん患者の精神症状に対して最もエビデンスが豊富なのは認知行動療法をはじめとした精神療法であり，最新のガイドラインでも薬物療法ではなく精神療法が推奨されている。終末期にはせん妄の頻度が高くなるが，病像に応じて薬物療法一辺倒ではない丁寧なケアが必要である。このような背景から，サイコオンコロジーの領域では臨床研究における患者市民参画（PPI）が先んじて取り入れられつつある。一方，課題も山積しており，がん患者の自殺対策，特別なケアが求められる世代としてAYAや高齢のがん患者，加えて同様に大きな心理的な衝撃を受ける家族・遺族への支援など喫緊のテーマも多い。本稿では，サイコオンコロジーの現状をわが国におけるがん対策の視点から俯瞰し，現在の課題を整理するとともに今後の精神医学をはじめとした精神保健分野に対する期待を展望した。
