胃腺腫の病理診断―特に胃型（幽門腺型）腺腫について

要旨　胃腺腫の代表的なものは異型上皮あるいは扁平腺腫と呼ばれ，小腸型の形質を発現することが多い．胃型の腺腫も発生するが，非常にまれなものとして扱われ，まとまった報告はなかった．今回，われわれは日独共同で幽門腺型腺腫を集積し，その特性を臨床病理学的・分子病理学的に検索した．幽門腺型腺腫は淡明～時に好酸性の胞体を有する背の低い円柱細胞からなる大小の管状腺管の密な増生する隆起性病変で，多くが胃上部に発生していた．腺管はMUC6強陽性で，表層被蓋上皮がMUC5ACの腺窩上皮型の形質を発現した．ペプシノーゲンIは陰性であった．したがって，この病変は胃底腺粘膜の幽門腺化生に関連して発生すると考えられる．一見異型性には乏しい病変であるが，粘膜内浸潤癌または高度異形成と言える病変を伴う症例が全体の20～30％程度存在した．また，5例を対象にしてcomparative genomic hybridization法を用いて染色体異常を解析したところ，4例で染色体異常を認め，うち3例では胃癌で一般的に証明されることの多い17p，17qと20qのgainを認めた．この病変の存在を念頭に置き，特に胃上部の半球状～結節状隆起性病変については，幽門腺型腺腫を鑑別診断の1つに挙げ，生検組織診で示唆された場合には積極的にpolypectomy/EMRによる診断的治療をすすめ，浸潤癌の存在の有無を調べる必要があろう．
