胃腺腫の病理診断―分子生物学的な解析

要旨　胃腺腫のEMR材料を用いて，腺腫と高分化型管状腺癌の鑑別目的に分子生物学的検討を行った．胃腺腫34病変および粘膜固有層に限局する高分化型管状腺癌28病変(低異型度癌19病変および高異型度癌9病変)で，HE標本で腺腫と診断した病変はいずれもp53陰性(p53陽性腫瘍細胞が25％以下)，かつcyclin E陰性(cyclin E陽性細胞50％以下)であり，p53陽性とcyclin E陽性の少なくともいずれか一方が認められた病変は，腺腫の可能性はなく，高分化型管状腺癌と診断可能であった．分子生物学的解析は従来の病理形態診断と相補的でかつ形態診断を裏付けるものであり，腺腫と高分化型管状腺癌の鑑別診断に有用である．
