難治性潰瘍性大腸炎に対する新しい治療―免疫抑制剤治療を中心に

要旨　潰瘍性大腸炎の診断・治療は研究班による診断基準案，治療指針案等の作成により，軽症や中等症に関しては専門医でなくても可能となってきた．しかし，いわゆるステロイド不応例，ステロイド離脱困難例などの“難治”と定義される症例は，現在でも治療に苦慮することが多く，結果としてステロイドが大量に投与されているのが現状である．これらの問題を解決するために，最近使用されることが多くなってきたのが免疫抑制剤である．例えば緩解導入療法としてはcyclosporineの持続静注療法が有効と報告され，当院の成績でも約70％に効果が認められ，その報告を裏付けている．また，緩解維持療法としてはAZAや6-MPの内服が有効とされ，今後，潰瘍性大腸炎における免疫抑制剤療法の発展が期待される．
