要旨　患者は68歳，男性．黒色便にて近医を受診し，消化管精査目的にて当院紹介となる．上・下部消化管内視鏡検査では異常を認めなかった．その後も黒色便を認め，貧血を呈したため，カプセル内視鏡を施行したところ，上部空腸に，潰瘍を伴う粘膜下腫瘍を認めた．ダブルバルーン内視鏡による生検組織の免疫組織学的検討でKIT陽性であり，GISTと術前診断し，空腸部分切除術を施行した．HE染色では紡錘形細胞が束状不規則に増生し，強拡大10視野中40個以上の核分裂像を認めた．免疫組織染色ではKIT陽性，CD34陰性，α-SMA陽性，NSE陽性で，筋原性・神経原性双方に分化を示す高悪性度のGISTと診断した．リンパ節転移は認めず，術後経過は良好である．
