米国における表面型大腸腫瘍の頻度と重要性

背　景

　現在，大腸癌の発癌経路としては多くの遺伝子異常の蓄積を伴って腺腫から大腸癌が発生するというadenoma-carcinoma sequenceが一般的であり，内視鏡的ポリペクトミーなど，日常診療にこの考え方が取り込まれている¹⁾．一方1985年にMutoらは通常のポリープとは異なった平坦な形態の表面型大腸腫瘍の存在を報告した²⁾．この表面型大腸腫瘍は癌の頻度が高く，大きさの小さなうちから粘膜下層へ浸潤することから既存の概念とは異なった新しい大腸癌の発育進展経路が示唆されるようになった．この表面型大腸腫瘍は日本においては広く受け入れられている概念であり，その発見は一般的となっており³⁾，旭川医科大学第3内科（日本）では全大腸腫瘍における表面型腫瘍の割合は21.3％（851/3,992）である⁴⁾⁵⁾．一方，欧米諸国の中でヨーロッパではこの表面型大腸腫瘍の概念は認められつつあり，その報告も増加している⁶⁾．スウェーデンから報告された表面型大腸腫瘍の頻度は24％であり，日本との差は認めない⁷⁾．一方，アメリカにおいては“表面型大腸腫瘍はアメリカでは極めてまれか，存在していても臨床的重要性は乏しい．癌の頻度については日本と欧米の病理診断基準が異なっているからであり⁸⁾，アメリカで行ったnationalpolypstudyの結果から考えても⁹⁾，表面型病変は少なくともアメリカでは大腸癌死亡には寄与していない”との見方がなされている¹⁰⁾¹¹⁾．
