19.放射線照射性腸炎

要旨　放射線照射性腸炎は婦人科領域や泌尿器科領域の悪性腫瘍に対する放射線治療後に発生するものであり，現在もなお重要な診療対象疾患の1つである．診断は放射線治療歴の聴取とともに手術痕や皮膚硬化の理学的所見が参考となる．診断は内視鏡検査が有用であり，毛細血管拡張，潰瘍，狭窄，瘻孔に留意する．治療は出血例には各種の薬剤による注腸療法や内視鏡的焼灼術が，狭窄例には内視鏡的拡張術が試みられている．内科的治療抵抗例に対する外科的治療は術後合併症の可能性を考慮して慎重な検討が必要であろう．長期経過例では癌誘発の可能性が生じるが，通常の癌と比較して形態や組織型に違いがみられるため，詳細な内視鏡観察が重要である．本疾患は，関係診療科の緊密な連携と長期間にわたる厳重な経過観察が必要である．
