下咽頭類基底細胞癌の1例

要旨　症例は73歳，男性．C型肝炎，肝硬変，肝細胞癌，食道癌にて2000年より経過観察中，2002年7月上部消化管内視鏡検査にて右下咽頭に病変を指摘．右梨状陥凹に表面比較的平滑，丈の高い丘状型の隆起性病変とその基部に正常血管網が透見できない，丈の低い隆起性病変を認めた．上皮下浸潤が疑われる病変であったが，進行性の他臓器癌を合併，全身状態不良のため，内視鏡治療を行った．病理では上皮下にmassiveに浸潤したbasaloid squamous cell carcinoma，10×9mm，ly₁，v₂であった．1年後リンパ節再発を認め，放射線治療を行ったが，肝細胞癌が増大し，内視鏡治療から22か月後死亡した．
