早期胃癌に対するESDと腹腔鏡下手術の接点―私はこう考える：外科の立場から

　予後の良い早期胃癌に対しては治療後のQOLの向上が求められるようになり，ESD（endoscopic submucosal dissection）や腹腔鏡下手術が急速に普及してきている¹⁾²⁾．ESDが治療後のQOL向上には最も良いが，治療法の選択においては，根治度，安全性，患者・医療者の負担や経済性などのバランスを考慮することが重要である．そこで早期胃癌に対するESDと腹腔鏡下手術の治療成績および問題点を評価し，早期胃癌の治療法選択についてわれわれの考えを述べる．

　これまでの詳細な臨床病理学的検討から早期胃癌のリンパ節転移状況は明らかとなり，リンパ節転移のない胃癌の条件として，①分化型M癌で潰瘍のないもの，②潰瘍のある分化型M癌では大きさ3cm以下のもの，③潰瘍のない未分化型M癌で大きさ2cm以下のもの，④3cm以下の分化型SM1癌で脈管侵襲のないもの，などが挙げられている³⁾．これらの病変はEMR（endoscopic mucosal resection）で根治できる可能性があるが，胃癌治療ガイドラインでは一括切除をEMRの原則とし，日常診療におけるEMRの適応を2cm以下の分化型M癌で潰瘍のないものとしている⁴⁾．しかし，大きな病変や潰瘍瘢痕合併例に対しても一括切除が可能なESDの出現により，その適応は徐々に拡大されつつある．
