センチネルリンパ節生検を用いた食道表在癌のリンパ節転移診断

要旨　食道表在癌に対する集学的治療策定において最も重要な因子はリンパ節転移の有無である．しかしながら，画像診断によるリンパ節転移診断の精度には限界があり，リンパ節転移の存在が否定しえない状況では大きな侵襲を伴う広範なリンパ節郭清が行われているのが現状である．センチネルリンパ節（sentinel node ; SN）は各種固形癌におけるリンパ節転移の初発部位として診断的価値が注目されている．食道癌においてもcT1N0症例においてはその有用性が報告されている．内視鏡的治療にて局所制御が可能な食道表在癌症例におけるSN生検による追加治療の適応決定，根治術症例における重点郭清部位の設定，SNを標的とした正確な転移診断に基づく補助化学療法の適応決定，化学放射線療法におけるSNを標的とした個別的照射野の設定など様々な臨床応用が期待される．食道癌を対象としたSN生検はいまだ克服すべき問題点を残しているが，食道表在癌の個別的集学的治療体系の確立に役立つ手法であると考えられる．
