要旨　腸管悪性リンパ腫は多彩であるが臓器特異性が存在する．びまん性大細胞型B細胞性リンパ腫（DLBCL）はどこでもかなりの頻度があるが，十二指腸は濾胞性リンパ腫が下行脚に好発するという特徴がある．空腸回腸ではT細胞性リンパ腫がしばしばみられ，臨床的に進行が速く予後不良例が多い．大腸では，MALTリンパ腫とDLBCLが主体である．前者は隆起性で一部では特異性の高いt(11；18)(q21；q21)がある．この異常を持つものは陰性例に比してBCL10の高発現とともに臨床病期が高い傾向があり，注意を要する．多臓器を侵すMALTリンパ腫について検討するとその大半が大腸病変を有している．消化管を広く侵す病態としてlymphomatous polyposisが知られているが，これはマントル細胞リンパ腫が主体であり，t(11；14)(q13；q32)に関与するcyclin D1高発現が診断的な価値が高い検査である．
