Helicobacter pyloriに起因しないとされる胃粘膜病変の病理所見

要旨　Helicobacter pylori（H. pylori）が発見され，H. pyloriが胃炎の主要因であることが明らかにされ，胃炎の考え方や分類は大きく転換した．しかしながら，胃炎および胃粘膜病変にはH. pyloriに起因しないとされる種々の非腫瘍性の粘膜病変が存在する．それらには，①胃炎関連病変：急性・慢性胃炎，特殊型胃炎（逆流性胃炎，化学物質・薬剤による胃粘膜傷害，肉芽腫性胃炎，感染性胃炎，好酸球性胃炎，collagenous gastritis），②腫瘍様病変（肥厚性胃炎，胃底腺ポリープ/ポリポーシス，特殊な消化管ポリープ/ポリポーシス，③その他（血管性病変，放射線性・抗癌剤性胃炎，アミロイドーシス，移植片対宿主病）などがあり，本稿でその組織学的特徴を概説した．今後，H. pyloriがそれぞれの疾患の増悪・軽快や癌化に及ぼす影響あるいはそれらの疾患がH. pyloriによる胃炎に及ぼす影響について解明していく必要があるが，まずこれらの組織学的特徴を理解しておくことが重要である．
