Crohn病の胃・十二指腸狭窄性病変に対する内視鏡的拡張術の有用性

要旨　炎症性腸疾患のうち，Crohn病（CD）では重大な消化管狭窄が多く出現する．腸管では既に内視鏡的拡張術が良好な成績を収めている．胃幽門部と十二指腸にも狭窄性病変が出現し，ときに手術適応とされる．しかし，低侵襲である内視鏡による拡張術の適応と効果は明らかでない．本論文では，胃・十二指腸狭窄に対する内視鏡的拡張術の短期および長期成績を検討する．対象は，11例の幽門狭窄症状を呈するCD患者．内視鏡的拡張術は10例で短期的に成功した．狭窄が長い例では短期的な拡張術が不十分であった．拡張術に起因する合併症はなかった．長期的（105か月観察）には，再発例を含めて拡張術後の手術回避率は1年で82％，3年で72％，5年で72％である．長期的成功ができなかった例の狭窄部位長は長い傾向にあったが有意ではなかった．内視鏡的拡張術は，手術を要する腸管狭窄の一部に対し有効で，長期成績も良好である．したがって本法がCDの胃・十二指腸狭窄に対する治療の標準的治療の1つになりうると考えられる．
