要旨　Helicobacter pylori（Hp）が胃癌の原因であることについて，疫学研究での知見に加え，動物実験や分子生物学的な研究の知見が加わったことから，この関係は確実である．Hpは胃癌のリスクを20～40倍にする．今後もデータの集積が必要であるが，成人での除菌の効果は胃癌のリスクを1/3にする程度である．除菌に成功しても感染したことのない人に比べ10倍程度リスクが高いことになる．Hpの主な感染時期は小児期である．有効な胃癌予防対策として，小児に接触する機会のある感染者を除菌して，小児への感染をブロックすることを検討すべきである．
