結節集簇様病変を呈した小腸進行癌の1例

要旨　患者は87歳，女性．上腹部痛を主訴に近医を受診し，腹部超音波検査にて小腸腫瘍が疑われたため当科紹介となった．小腸X線検査，およびダブルバルーン内視鏡検査では空腸に丈の低い全周性の結節集簇様病変が認められ，その中心部では結節状隆起と狭窄を伴っていた．生検組織所見では高度異型腺腫と診断されたが，臨床的に小腸進行癌と考え外科的治療を選択した．術中内視鏡では病変肛門側にも粗大結節の集簇と丈の低い扁平隆起がみられた．切除標本の肉眼所見では大小不同の結節が集簇した病変で，組織学的には大部分は種々の異型度を呈する腺腫であり，その中心部で粘膜下層および固有筋層まで浸潤した癌がみられた．しかし，癌と腺腫の境界は不明瞭であった．自験例は腺腫が結節集簇様病変を呈し進行癌へ進展したと考えられた．
