多発GISTの臨床病理学的特徴

要旨　GISTのほとんどは単発性に発生するが，極めてまれに多発性GISTに遭遇することがある．それらには，通常の散発性に発生するGISTの他，c-kit，PDGFRA遺伝子の胚細胞変異をもつ家族性GIST，神経線維腫症1型に伴って発生する多発GIST，傍神経節腫を合併するCarney triad，Carney-Stratakis dyadに伴うGISTなどが含まれていると考えられ，それぞれの腫瘍発生にかかわる遺伝子的背景は多様である．したがって適切に分子標的治療薬を使用するためにも，これら多発性GISTに遭遇した際には，患者，遺伝子的背景に基づいてGISTを亜型分類しておくことが必要であると考える．
