要旨　最近，Crohn病を含めた小腸疾患は増加傾向にあり，手術症例数も増加している．本稿ではCrohn病に対する腹腔鏡下手術の現状と適応について概説する．Vienna分類を用いて，合併症，開腹移行について検討した．Lap（腹腔鏡下手術群）ではB3L3/4の総合併症，major合併症，開腹移行は，それ以外のグループより有意に高率であった．単変量解析ではB3L3/4は有意に開腹移行と相関していた．またB3L3/4は，多変量解析で総合併症の独立した危険因子であることが示された．次にB3L3/4においてLapとOpen（開腹手術群）を比較したところ，総合併症，major合併症ともに差を認めなかった．以上から，B3L3/4以外のグループでは腹腔鏡下手術はよい適応で，外科手術の第一選択となりうる．また，B3L3/4の患者に対しては，腹腔鏡下手術は安全に施行可能であるが，開腹移行率が高いことに留意すべきである．
