消化管follicular lymphomaの治療方針―私はこう考える

はじめに

　follicular lymphomaは，悪性リンパ腫の中で，腫瘍の増大速度が遅く，比較的緩徐な経過をたどり，全身リンパ節に進展して発見される例が多く，I期，II期の限局期に発見されるものは少ない¹⁾．しかし，消化管においては，内視鏡検査施行時に，十二指腸に限局した白色顆粒集簇病変として偶然発見されるなどして，I期，II期の限局期に発見される頻度が高い．一方，限局期のfollicular lymphomaに対する治療は放射線療法が標準的治療であるが，rituximab併用化学療法の開発により，治療の選択も拡大しつつある．ここでは，そのような現状をふまえ，消化管follicular lymphomaに対する治療について述べたい．
