はじめに

　十二指腸原発濾胞性リンパ腫（follicular lymphoma；FL）は，消化管リンパ腫の約1～5％と言われており，Vater乳頭部付近に多発性ポリープ様病変を呈すること（Fig. 1 a）が多い．近年十二指腸原発FLは増加しているが，検診などで偶然見つかることも多く，この疾患が広く認識されたことによる内視鏡医や病理医の診断能力の向上が一因と考えられる．節性FLの治療は，rituximabの臨床応用により大きく変化したが，十二指腸原発FLに対する治療戦略は確立されていない．今回，血液腫瘍医の立場から治療法について考えてみたい．
