要旨　潰瘍性大腸炎術後に発症する回腸囊炎の診断には，診断基準にのっとった臨床および内視鏡所見の適切な評価が求められる．厚生労働省難治性炎症性腸管障害調査研究班（日比班）で作製した回腸囊炎診断基準は，内視鏡所見による重症度を基本に臨床所見を加味したもので，PDAI（Pouchitis Disease Activity Index）との互換性にも配慮している．診断に際し，手術操作により排便状況が症例に応じて様々に変化する可能性のあること，外科的合併症の有無，残存直腸粘膜の有無などの個別性を念頭に置く必要がある．回腸囊炎の“初期病変”の解析はほとんど行われておらず，解明には経過を追った内視鏡観察が求められる．
