日本のcollagenous colitis―まれな疾患か見落とされやすい疾患か

はじめに

　本誌でCC（collagenous colitis）^1）の特集を組むのは今回が初めてである．欧米ではCCの報告例が多いのに比し，本邦ではCC症例はまだ少なく，最近になり報告例が増加しつつある状況である^2）～5）．本症は，診断が困難であるため疾患としての認知が本邦では遅れていた．早期胃癌研究会でも，数年前から数例が提示されており，筆者も読影に指名されたが正しく診断できなかった．以前に数例の自験例があったにもかかわらず^6）7），診断ができなかったのである．診断が困難な理由は，まれであるため日常の鑑別診断に挙げられないことが大きい．それに加え，後から内視鏡画像を見ても著しい所見，あるいは特異的な所見を挙げ難いこともある．

　しかし最近は，CC症例が研究会にも頻繁に提示されるため，臨床像と合わせて総合的に考えると鑑別に苦しまなくなった．各施設とも10例を超える自験例を経験していると思われる．最近の報告例を見ると，本邦例の臨床的な特徵（性，年齢，症状など）が海外の報告例と共通する点が多い．一方，かけ離れている点もみられ，その実態に大変興味をひかれる．なかでも本邦では，CCの臨床的な特徴として薬剤との関連が強いと考えられているが，どうしてであろうか．また，欧米ではCCは内視鏡的に異常がないと言われているが，CCの本場スウェーデンに留学した医局員に聞くと，細かく内視鏡所見を検討すると本症にはいろいろな所見があり，内視鏡診断が可能な例も多いとの情報もある^7）8）．今後，日本においてもCCに対しての内視鏡的な診断と鑑別能が向上することを期待したい．

　思い起こせば20年以上前，共同研究者の飯田三雄先生（九州大学大学院病態機能内科学）と虚血性大腸炎を調査している際に横行結腸に長大な縦走潰瘍例を見た．そのとき，飯田先生が“この例は通常の虚血性大腸炎とは臨床的・形態学的にかけ離れている”と断じ，対象例に組み入れなかったことを記憶している^9）．その特徴的な例は現在の情報から考えると，CCが基礎疾患として存在したと推測される．最近，縦走潰瘍を伴う例を見るたびに思い出されるエピソードである．

　CCの診断基準は海外のものを使用するしかない．それによると，CCはLC（lymphocytic colitis）と類縁疾患であり，両疾患を合わせてMC（microscopic colitis）と総称される^{10）～13）}．しかし，本邦ではCCの報告は散見されるが，LCの報告例はほとんど見当たらないようである．この点は，本邦の特徴であろうか，あるいは診断の遅れが原因であろうか．MCの中でも，CCは最近急に認知度が高まり，多数例の検討も始まったが，LCは病理像さえ広く認知されているとは言い難い．そこで本号ではLCは取り上げず，CCのみを検討対象とした．本企画によりCCの診断と理解が進み，LCについては次回には特集が組まれるくらい情報が集積されることを強く期待する．
