小さな薬剤起因性小腸病変―NSAIDsによる小腸病変

要旨　nonsteroidal antiinflammatory drugs（NSAIDs）は，慢性疼痛の治療のみでなく，脳梗塞や心筋梗塞後の再発予防や，不整脈による血栓症予防に対して広く使用されており，上部消化管のみでなく下部消化管，特に小腸にも粘膜傷害を来す．従来のX線造影検査では微小病変の描出が非常に困難であり，小腸病変の診断は容易ではなかったが，近年カプセル内視鏡およびダブルバルーン小腸内視鏡検査が臨床使用されるようになり，微小病変を含んだNSAIDsによる小腸病変の病像の解明がなされている．NSAIDs小腸病変の動物モデルでは縦走潰瘍を来すことが知られているが，ヒトにおいてはさらに輪状潰瘍，膜様狭窄，円形打ち抜き状潰瘍，アフタ様潰瘍，びらんなどの病変が認められる．
