要旨　腸上皮化生（intestinal metaplasia ; IM）の存在を含めた背景粘膜の形質と胃癌の発生との関連について，様々な状態の胃における解析結果と文献報告を総合して考察した．微小胃癌および同時性多発胃癌の解析結果から，分化型癌はIMを背景として発生するのではなく，癌が発生する状況ではIMも発生しやすい，すなわちIMは前癌病変というより傍癌病変であると考えられた．IMを伴わない胃粘膜に発生した胃癌や若年者胃癌の解析からは，未分化型癌の発生においては，少なくともIMは癌化の母地として重要ではないことが示された．また，良性病変切除後の残胃癌ではIMは癌化と無関係で，逆流性胃炎の存在が重要であった．胃癌切除後の残胃に発生した癌（特に10年以内に発生）は同時性多発胃癌と同様にIMとの関連があった．過形成ポリープに癌が発生する場合も過形成上皮におけるIMは癌化とは無関係であった．以上より，背景胃粘膜のIM（腸型形質発現）は前癌病変とは考えにくいが，その出現は分化型癌発生の危険因子として意義があり，IMを伴わない粘膜に発生する胃癌ではH. pylori感染による炎症や腸液の刺激が癌化の危険因子として意義があると考えられる．背景粘膜の癌発生の危険性を判定する場合には，これらの因子（IM，炎症，腸液の刺激など）の影響を総合的に評価する必要があると思われる．
