内視鏡による早期胃癌のIIb進展範囲診断―AIM法を用いた色素内視鏡診断法を中心に

要旨　内視鏡的粘膜下層剥離術（ESD）の普及に伴い早期胃癌の進展範囲診断の重要性が増している．ESDは任意の範囲の一括切除が可能であり，理論的には切除可能な病変の大きさに制限はない．そのため，ESDにおいては厳密な病変の進展範囲診断が必要である．特にIIb型早期胃癌は範囲診断が難しく，さらなる診断技術の向上が必要である．現在新しい内視鏡診断法として，image-enhanced endoscopy，拡大内視鏡診断などが試みられているが，筆者らは簡便かつ有用な試みとして酢酸インジゴカルミン混合液による色素内視鏡（AIM法）を考案し，その有用性を報告してきた．今回，AIM法による診断を中心にIIb型早期胃癌進展範囲診断について概説する．
