遺伝性消化管疾患の特徴と長期経過

　遺伝性を有する消化管疾患として，遺伝性ポリポーシス症候群（hereditary polyposis syndromes ; HPS）と遺伝性非ポリポーシス大腸癌（hereditary non-polyposis colorectal cancer ; HNPCC）がよく知られている（Table 1）．前者は組織学的に腺腫性，過誤腫性，過形成性の3種類に分類されるが，臨床的には家族性大腸腺腫症（familial adenomatous polyposis ; FAP）が最も重要である．FAP以外の腺腫性ポリポーシスとして，MYH（mutY homolog）関連大腸腺腫症（MYH-associated polyposis ; MAP）やTurcot症候群が挙げられる．過誤腫性ポリポーシスには，Peutz-Jeghers症候群，Cowden病，若年性ポリポーシスが含まれるが，近年，結節性硬化症もこの疾患群に包含されている．また，極めてまれではあるが，過形成性ポリポーシスが家族内に集積する病態も報告されており，家族性過形成性ポリポーシス（familial hyperplastic polyposis）と呼ばれている．

　一方，HNPCCはLynch症候群とも呼称され，若年発症，右側大腸癌，大腸多発癌，多臓器重複癌を臨床的特徴とする遺伝性疾患である．そのほか，HPSにもHNPCCにも包含されない遺伝性消化管疾患として，Blue-Rubber-Bleb-Nevus症候群，Rendu-Osler-Weber病，家族性消化管腫瘍などが知られている．
