要旨　CTC（CT colonography）は，仮想内視鏡（virtual colonoscopy）とも呼ばれ，CTのスライスデータから構成された大腸内腔の三次元表示法である．CTCは，大腸癌のスクリーニング法として欧米を中心に研究され，最近の米国での大規模臨床試験ではポリープの検出能が内視鏡検査に匹敵すると報告されるなど，さらなる臨床活用へと普及している．本邦では大腸癌による死亡率が年々上昇し，FOBT（fecal occult blood test）の早期癌に対する感度の低さ，さらにFOBTで陽性と診断されても大腸内視鏡検査による精密検査への受診率が低いことなどから大腸癌スクリーニング法の改良が求められている．そこで，CTCは術者の技量に関係なく安全かつ簡便に施行可能であり，さらに検査処理能力に優れ，客観性に富む診断画像として大きく期待されている．現在，CTCの大きな方向性として，“いかに被検者のCTCへのコンプライアンスを高めるか”，“いかにCTCの検出精度を高め，かつ読影時間を短縮できるか”である．そこで筆者らは，CTCの基本を概説し，引き続き最近注目されているfecal tagging（残渣マーキング），electronic cleansing（電子洗浄法），CAD（computer-aided detection，コンピュータ支援検出）の現状と将来展望について述べる．
