要旨　筆者らが新しく提唱した胃癌の組織亜型・胃底腺型胃癌27病変（25患者）の臨床病理学的特徴，発育進展様式，生物学的悪性度に関して再評価した．胃底腺型胃癌は，高齢者のU領域に好発するがM領域にも存在し，胃炎のない正常胃底腺粘膜の深層部から発生し，初期には0-IIa/0-IIc型の形態をとり，小さいものでも高率にSMへ浸潤する．そして，大きくなると0-IIa＋IIc型のような複合型の形態をとるものや，表層方向への増殖により隆起（0-I型）を来すものがある．悪性度に関しては，特に小さいものでは低い細胞異型，脈管浸潤陰性，低い増殖活性，p53蛋白過剰発現なし，予後良好ということより，悪性度は低いと考えられるが，比較的腫瘍径が大きく異型がより高度のものでは，脈管浸潤やCD10陽性成分が出現しているものもあり，進行するにつれて異型度や細胞分化の変化が生じ，高悪性度へと変化する可能性が示唆された．
