Barrett食道癌の内視鏡診断―通常観察での拾い上げ診断のポイント

要旨　Barrett食道癌の拾い上げのためには通常内視鏡検査でよく観察することが大切なのは言うまでもない．日本においてはSSBEから発生するBarrett食道癌が多いので，特に食道胃接合部をよく観察しなければ，微小なBarrett食道癌を拾い上げることは困難である．当科で経験した表在型Barrett食道癌は，0～2時方向，すなわち前壁～右壁に存在することが多く，色調は8割以上の症例で発赤調を呈していた．よって，Barrett粘膜内の0～2時方向に存在する発赤部位に注意することが重要である．さらに，逆流性食道炎治療後に明らかとなるBarrett食道癌があること，生検後に病変が扁平上皮に覆われ診断が困難となるBarrett食道癌が存在することも認識しておく必要がある．
